What do I need to bring?
Please bring any recent blood work or other medical test results as well as the bottles of all supplements you are currently using. (If purchased at Freshlife - there is no need to bring the bottles since we will have access to label details)
What is the fee for the consultation?
$40 for the initial consultation and $15 for follow-up visits
When/where are the consultations held?
At the Freshlife Wellness Center, 2304 East 3rd Street. This is the building just east of the store.
Who schedules the appointments?
Your Wellness Coach will contact you to set up your appointment.
How do I get the intake form?
By stopping by the store and asking at the checkout ($40 fee), or submitting our online intake form. If you wish to have a form mailed to you, please send $40 along with an SASE to: Freshlife 2300 E Third St. Williamsport, PA 17701
Are consultations available long distance?
Yes, consultations are available by phone. The health history must first be submitted. You will be contacted regarding payment arrangements prior to the time of your appointment.
Can I bring a family member/friend with me when I come for the consultation?
Certainly, that is your choice.
Do you do consultations with children?
We do consultations with parents on behalf of children under 10. The health history is still necessary. We are happy to meet with the children and parents when the child is over 10.
Do you do consultations with pets?
Pet consultations are available at the Wellness Center through Earth Dogz. Melian Boyle, CVT offers this service. You may contact Earth Dogz at 570-772-5504.
I have a strange work schedule, can I get a weekend or late evening appointment?
Please discuss your scheduling needs with your Wellness Coach as special arrangements can sometimes be made.
How long is the waiting list for appointments?
Generally about 2 weeks from the time you submit the form.
What if I have to cancel?
Cancellations are inevitable, and we accept that. Of course, we would prefer as much notice as possible. There are no refunds for a client who misses a scheduled appointment without prior notification.
